For the first time, viewers are seeing a new angle of the midair collision involving American Airlines Flight 5342 and a U.S. Army Black Hawk helicopter over Washington, D.C.—a crash that claimed 67 lives.

In a rare on-camera moment, an air traffic controller involved in the incident summed it up in chilling terms: “It worked… until it didn’t.”

The newly surfaced footage and firsthand account are raising fresh questions about how such a catastrophic failure could occur in one of the most tightly controlled airspaces in the country.
